Therapy at Passfield Park School
All therapists are required to provide the school with a signed letter of consent from the parents or carers before any communication can take place regarding a child. The consent to exchange information needs to be specific, include the child’s full name, your full name, your organisation and be signed.
In additional to the signed consent to exchange information, families that would like therapy to take place at school, will need to complete and sign the below NDIS Funded Therapy Request form and provided this to the school. Therapy goals need to be clearly defined with collaboration from parents/carers, therapists and the school.

All documents should be emailed to passfield-s.school@det.nsw.edu.au
Once all documentation is received, a meeting will be arranged between the principal and therapist to discuss how and when therapy will be delivered at school. External providers will be required to attend the Personalised Learning and Support Plan (PLSP) meeting for the student prior to commencing therapy at school. If this is not possible, a meeting must be held with the class teacher to discuss the student's current goals.
